This Friday!!! August 4th, 10am-2pm
Does my athlete need to turn in a Physical Examination Form or an Interim Questionnaire Form?
Athletes are required to turn in new or updated Physical Examination Forms if entering 6th and 9th grades or are participating in TVCA athletics for the first time regardless of the grade.
6th-8th Grade: TVCA Health and Physical Examination Form
9th Grade: IHSAA Health and Physical Examination Form
Athletes are required to turn in an Interim Questionnaire in lieu of a Physical Examination Form if they are entering 7th or 8th grade and already have a previous Physical Examination Form on file.
